Qualifications

• Massachusetts PT or OT license required.

• Reliable transportation to attend client visits.

Responsibilities

• Conduct in-home physical or occupational therapy assessments.
• Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs).
• Provide hands-on therapeutic interventions and progress monitoring.
• Educate clients, families, and caregivers on safe transfer techniques and home modifications.
• Maintain clear, reliable communication with families and our clinical coordination team.
