Qualifications

• Active Massachusetts PT or OT license required.

Responsibilities

• Conduct in-home physical or occupational therapy assessments.
• Develop and implement personalized care plans focusing on mobility and ADLs.
• Provide hands-on interventions and monitor progress.
• Educate clients, families, and caregivers on safe transfer techniques and home modifications.
• Maintain clear communication with families and our clinical coordination team.

Your Core Responsibilities:

• Educate clients, families, and caregivers on safe transfer techniques and home modifications

What We Are Looking For:
