Qualifications

• Active Massachusetts Physical Therapist (PT) or Occupational Therapist (OT) license required.
• Experience in home health or rehabilitative settings preferred.
• Strong communication, documentation, and teamwork skills.

Responsibilities

• Conduct comprehensive in-home physical or occupational therapy assessments.
• Develop and implement personalized care plans focused on mobility, safety, and ADLs.
• Provide hands-on therapeutic interventions and progress monitoring.
• Educate clients, families, and caregivers on safe transfer techniques and home modifications.
• Maintain clear, reliable communication with families and our clinical coordination team.
