Boost Home Healthcare in Greater Boston is seeking a licensed Physical Therapist to provide intermittent home-based care. You will visit patients' homes, assess mobility, and design personalized plans to improve strength, balance and independence.

Expect collaboration with a care team and well-supported field visits. You’ll work under the Director of Clinical Services or Clinical Supervisor, delivering physician-ordered therapy while maintaining Medicare OASIS documentation and quality care.

Qualifications

• Degree in physical therapy from a CAPTE-accredited program.
• Current Massachusetts PT license.
• 2+ years of home health experience preferred.
• Working knowledge of Medicare and OASIS documentation.
• Comfortable working independently in the field with reliable transportation.

Responsabilités

• Travel to patients' homes to perform initial evaluations and develop individualized plans of care.
• Deliver physician-ordered physical therapy to restore function, mobility and independence.
• Teach patients and caregivers home exercise programs and home safety strategies.
• Reassess patients at least every 30 days and update the plan of care.
• Complete timely Medicare OASIS and visit documentation.
• Coordinate care with nurses, therapists, social workers and physicians.
• Supervise therapy assistants as required.
