Job summary

Creative Solutions Services, LLC is seeking a Physical Therapist for an in-home outpatient setting in Lancaster, NH. The 17‑week contract runs 40 hours per week, 8 hours per day, onsite with a structured schedule.

Qualifications

• Must have a valid Physical Therapy license in the state of practice or license eligibility.
• A Master's degree in Physical Therapy or a related field with a Bachelor's in Physical Therapy and 5+ years of PT experience is acceptable.

Responsibilities

• Provide outpatient rehabilitation services in home settings to older adults.
• Evaluate, plan and implement individualized physical therapy programs.
• Educate patients and caregivers to promote independence and safety at home.
• Supervise assistants and students as required by state licensure.
• Maintain timely documentation and adhere to schedules.
