Job summary

Creative Solutions Services, LLC seeks a Physical Therapist for outpatient rehabilitation in home and facility settings in York, PA. The role is a 13-week onsite contract starting 12/01/2026 with a 40-hour guarantee and 8:00 AM–4:30 PM shifts.

Responsibilities include evaluating and treating patients, coordinating with the rehab team, and educating patients and caregivers to promote independence and safe home mobility.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy or a related field; or
• Bachelor's degree in Physical Therapy plus 5 years of PT experience.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students and support personnel as per state licensure requirements.
• Adhere to department and patient treatment schedule and complete timely documentation.
• Attend patient care and staff meetings and contribute to planning.
• Refer to other disciplines to address comprehensive patient needs.
• Collaborate with rehabilitation team to select optimal therapy regimens.
• Adapt plan of care to patient needs and ensure safety in the home environment.
• Order supplies and maintain/evaluate equipment as necessary.
• Deliver high-quality patient service in a compassionate setting.
• Perform patient evaluations, develop goals, and progress plans with patients and caregivers.
