Responsibilities

• Provide outpatient rehabilitation services in a home setting to older adults.
• Evaluate, plan, and deliver PT programs to restore function and promote independence.
• Educate patients and caregivers in skills essential for independent living.
• Supervise PT assistants and students as allowed by state licensure.
• Maintain timely documentation and comply with scheduling and compliance standards.
• Collaborate with rehabilitation team to coordinate care and referrals.
• Order supplies, maintain equipment, and ensure patient safety.

Education

PT license

Master’s degree in Physical Therapy

Master’s in PT related field + Bachelor’s in PT

Bachelor’s in Physical Therapy + 5 years PT experience
