Job summary

Metro Physical & Aquatic Therapy is seeking a motivated and compassionate full-time Physical Therapist to provide in-home care across Eastern Suffolk County, NY. The role offers a base salary of $95,000–$110,000 per year plus a $3,000 sign-on bonus and a comprehensive benefits package.

All levels of experience are welcomed; new graduates will be mentored, and travel to patients' homes within the geographic territory is required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in the state of New York or eligible for licensure.
• Home health experience is preferred but not required — we will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare, and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
