Job summary

Metro Physical & Aquatic Therapy is seeking a full-time Physical Therapist to provide in-home care across Eastern Suffolk County, NY. The role offers a competitive base salary and a $3,000 sign-on bonus, with a focus on patient-centered care and flexible scheduling.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in New York or eligible for licensure.
• Ability to travel to patients' homes within the assigned geographic territory.
• Home health experience preferred but not required; mentor provided for documentation/compliance.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs and develop individualized home-based treatment plans.
• Plan and implement customized therapy programs to restore function and independence.
• Instruct family members and aides on therapy techniques and home exercises.
• Provide timely documentation for each visit and coordinate with physicians.
