Job summary

Metro Physical & Aquatic Therapy in Home Care is seeking a motivated in-home Physical Therapist to serve Eastern Suffolk County on a full-time basis. A base salary range of $95,000–$110,000 per year plus full benefits and a $3,000 sign-on bonus.

Responsibilities include evaluating patients at home, developing individualized plans of care, coordinating with physicians and families, and delivering evidence-based therapy with flexible scheduling across the designated geographic area.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in New York or eligible for licensure.

• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

The anticipated base salary range for this position is $95,000–$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
