Job summary

Metro Physical & Aquatic Therapy seeks a full-time Physical Therapist to provide in-home care across Eastern Suffolk County, including Medford and Patchogue. Base salary ranges from $95,000 to $110,000 per year, with a $3,000 sign-on bonus and full benefits.

The role emphasizes flexible scheduling, autonomy, and opportunities for professional growth within a supportive, innovative team. All experience levels are encouraged to apply, with training provided for new graduates.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in NY or eligible for licensure.
• Home health experience is preferred but not required; mentor provided for documentation/compliance.
• Ability to travel to patients' homes within assigned geographic territory.

Responsibilities

• Accept and treat patients within the designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care with physician, patient, and family.
• Plan and implement customized therapy programs to restore function.
• Instruct families and aides on therapy techniques and ensure proper documentation.
