Green Key Resources is seeking a Psychiatric Nurse Practitioner to join an Assertive Community Treatment (ACT) Team in Brooklyn, NY. This field-based role delivers psychiatric care to individuals in their homes and community settings.

The ideal candidate will have NYS NP licensure, ACT experience, and comfort with home visits. Travel across Brooklyn and collaboration with an interdisciplinary team are essential.

Qualifications

• Active New York State Psychiatric Nurse Practitioner license and certification.
• At least 1 year of experience as a Psychiatric Nurse Practitioner.
• Solid understanding of the ACT model.
• Willingness to conduct field visits and travel within Brooklyn.

Responsabilités

• Conduct psychiatric evaluations and medication management.
• Provide in-home and community-based psychiatric care across Brooklyn.
• Collaborate with an interdisciplinary ACT Team to coordinate patient care.
• Support individuals with serious mental health conditions and complex needs.
• Develop and manage treatment plans to promote stability and recovery.
• Coordinate with clinicians to ensure access to services and engagement in treatment.
