Job summary

CASES is seeking a Psychiatric Mental Health Nurse Practitioner in the Bronx to deliver ACT services through a mobile, multi-disciplinary team. You will evaluate, treat, and monitor adults with serious mental illness and co-occurring substance use, coordinating care with providers and housing services.

The role offers full-time hours (Mon-Fri, 9am-5pm) with flexible work options. A strong background in community-based mental health and trauma-informed care is required.

Qualifications

• Licensed as a Psychiatric Nurse Practitioner by NYS Education Department.
• Board certification by AANP or ANCC.
• Extensive experience with adults with serious mental illness and co-occurring issues.

Responsibilities

• Conduct initial psychiatric evaluations and monthly assessments in the community.
• Prescribe medications via e-prescribing and monitor responses.
• Coordinate medication management with FACT nursing staff and related tests.
• Complete assessments and documentation per OMH, DOHMH, and Medicaid guidelines.
• Coordinate care with family, housing providers, hospitals, and other service providers.
• Participate in team and clinical meetings to discuss client needs.
• Utilize motivational interviewing and trauma-informed approaches in service delivery.
• Perform other duties as assigned by ACT leadership.
