Job summary

Signify Health, part of CVS Health, is seeking a Part Time Physician to perform in-home health evaluations using an iPad and a standardized clinical workflow. Each visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and, when required by the health plan, additional diagnostic tests.

As a part‑time clinician, you will travel locally to members’ homes, may conduct virtual visits, and provide Diagnostic & Preventive Services as indicated.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Multi-state licenses (preferred)

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Be flexible to travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
