Job summary

Porter Cares is expanding into New Jersey and seeks Registered Nurses to conduct in-home assessments across Hudson and Bergen counties. You will visit members, review medical history, perform vital signs, medication reconciliation, and a comprehensive exam to support care management.

The role requires a valid unencumbered New Jersey NP license, at least 3 years of patient care experience, and excellent communication with patients and colleagues.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience in primary/adult/geriatric settings; EMR skills preferred.
• Exceptional communication skills with patients and coworkers.

Responsibilities

• Visit members within the geographic area and conduct in-home health risk assessments.
• Evaluate past medical history, review systems, reconcile medications, record vitals, and perform a comprehensive exam.
• Support care management with Porter Care Guides and active medical management.
