Job summary

Porter Cares is expanding into New Jersey and seeks Registered Nurses to perform in-home health assessments and medical management in assigned counties. You will travel within Morris, Passaic, Sussex, and Warren to evaluate patient histories, vital signs, and medications, contributing to personalized care plans.

Ideal candidates hold a New Jersey NP license with 3 years of patient care experience and strong empathic, communicative skills.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience required (primary care/adult/geriatric).
• Exceptional communication and empathy toward patients and coworkers.

Responsibilities

• Visit members within your geographic area to perform in-home health risk assessments.
• Conduct comprehensive physical exams, review medical history, and reconcile medications.
• Support care management through Porter Care Guides and active medical management.
