Job summary

Porter Cares in New Jersey is expanding its in-home nurse practitioner program. You will visit members in Hudson and Bergen counties to perform health risk assessments, review histories, and conduct comprehensive exams in the home setting.

The role requires a valid NJ NP license, at least 3 years of patient care experience, and a compassionate, communicative approach. You will be reimbursed travel mileage and enjoy flexible scheduling.

Qualifications

• NJ NP license to practice in New Jersey.
• Minimum 3 years of patient care experience in primary/geriatric settings; EMR skills preferred.
• Demonstrated empathy, compassion and strong communication with patients and colleagues.
• Willingness to travel within Hudson and Bergen counties to visit members.

Responsibilities

• Conduct in-home health risk assessments including review of medical history, medications, vital signs and physical examination.
• Identify diagnoses for care management and coordinate with Porter Care Guides and medical management teams.
• Document findings and support development of current and past medical condition lists.
• Provide compassionate, patient-centered care and clear communication with patients and family members.
