Job summary

Porter Cares is expanding into New Jersey and seeks experienced Nurse Practitioners to perform in-home health risk assessments and comprehensive exams for our members. You will evaluate medical histories, reconcile medications, and document vital signs to support care management with Porter Care Guides.

The role requires an active New Jersey NP license, 3 years of patient care in primary care/adult/geriatric settings, and strong empathy plus communication skills for patients and coworkers.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience in primary care/adult/geriatric.
• EMR systems experience preferred.
• Strong empathy and communication skills with patients and coworkers.

Responsibilities

• Visit members in their homes within the geographic area and perform in-home health risk assessments.
• Evaluate medical history, review systems, reconciliation of medications, and vital signs.
• Conduct comprehensive physical exams and assist in care management planning with Porter Care Guides.
• Identify diagnoses for care management based on findings and clinical judgment.
