Job summary

Porter is expanding into New Jersey and is seeking Registered Nurses to join our in-home care team. You will visit members in your geographic area, perform risk assessments, review medical history, and coordinate care with Porter Care Guides to support personalized treatment plans.

Requirements include a valid unencumbered New Jersey NP license, 3 years of patient care experience (EMR skills preferred), and excellent communication and compassionate bedside manner.

Qualifications

• Valid unencumbered New Jersey NP license.
• 3 years of patient care experience required (primary care/adult/geriatric).
• EMR skills preferred.
• Strong empathy, compassion, and communication skills.

Responsibilities

• Visit members within your geographic area for in-home risk assessments.
• Evaluate past medical history, review systems, reconcile medications, obtain vital signs, and conduct comprehensive exams.
• Collaborate with Porter Care Guides to formulate care plans and coordinate medical management.
