Job summary

Ennoble Care in Hagerstown, MD seeks a Part Time Advanced Practice Provider (NP/PA) to deliver primary care in patients’ homes within Washington County. The role requires MD-NP/PA board certification, 1–2 years of primary care, and geriatric experience is preferred.

Qualifications

• Board Certified NP or PA in Maryland.
• 1–2 years of primary care experience.
• Geriatric care experience preferred.

Responsibilities

• Provide primary and palliative care in patients' homes around Washington County.
• Perform physical exams and develop treatment plans.
• Communicate with office staff across internal platforms.
• Deliver patient-centered care to a defined panel.
• Develop impressions, diagnoses and management plans.
• Consult with patients, families and caregivers during visits.
• Order diagnostic tests as needed and manage medications.
