Lucethealth in Phoenix, AZ is seeking qualified providers to deliver in-home and community-based care across Maricopa County. You will practice medicine outside the traditional clinic setting with clinical autonomy and a strong interdisciplinary support team.

Responsibilities include at least 25 in-home visits per week, chronic condition management, patient education, and thorough EMR documentation while complying with HIPAA.

Qualifications

• Active, unrestricted current licensure as a Provider in the state where you will practice.
• Certification as FNP, AGNP, or PA.
• DEA license and BLS certification.
• Arizona market: 1+ year of primary care or related field.
• MVR with reliable transportation.
• Must pass comprehensive background checks.

Responsabilités

• Conduct in-home visits (minimum 25/week).
• Manage chronic conditions and create individualized treatment plans.
• Serve as primary care provider with follow-ups.
• Maintain accurate EMR documentation and HIPAA compliance.
• Educate patients on disease management and healthy living.
