The Judge Group is seeking a dedicated Provider (FNP/AGNP/PA) to deliver comprehensive primary care to patients in their homes and in community-based facilities within Maricopa County, AZ. The role focuses on improving patient outcomes, continuity of care, and supporting behavioral, physical, and social health.

Responsibilities include in-home visits, chronic condition management, patient education, and collaboration with interdisciplinary teams while ensuring HIPAA-compliant EMR documentation.

Qualifications

• Active, unrestricted licensure as NP or PA in the practicing state.
• Credential as FNP, AGNP, or PA with current board certification.
• Ability to obtain autonomous provider status within six months of eligibility.
• DEA license and BLS certification.
• Valid driver's license and reliable transportation with a clean driving record.
• High-speed home internet connection.
• Minimum 1 year of primary care or outpatient experience.

Responsibilities

• Perform in-home visits (≥25 per week) with exams and medication reviews.
• Manage chronic conditions and develop individualized care plans.
• Act as primary care provider with follow-up visits.
• Educate patients on disease management and healthy living.

• Conduct in-home visits (minimum 25 per week), including assessments, exams, and medication reviews.
• Manage chronic conditions and develop individualized treatment plans.
• Serve as the primary care provider and conduct regular patient follow-ups.
• Educate patients on disease management and healthy living.
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality care.
• Maintain accurate EMR documentation and comply with all healthcare and HIPAA regulations.

Required Qualifications

• Active, unrestricted current licensure as a Provider (NP or PA) in the state of practice, clear of any clinical or board restrictions.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Physician Assistant (PA).
• Ability to obtain autonomous provider status within six months of meeting eligibility requirements.
• DEA license and BLS certification.
• Valid driver's license, reliable transportation, and a clean driving record (MVR).
• High-speed internet service at home
• Ability to pass a comprehensive background check and 10-panel drug screen.
• 1+ year of experience in primary care, family practice, general practice, or outpatient care.

Experience with:

• Risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
• Electronic Medical Records (EMR) documentation.
• Independent clinical decision-making and in-home assessments.

Schedule Requirements

• Ability to maintain a highly mobile schedule traveling regularly to patient homes and provider facilities.
• Flexibility to cover a territory radius of up to 50 miles from your home address within Maricopa County, AZ.
