Job summary
Monogram Health seeks Advanced Practice Providers (NP/PA) to deliver in‑home, evidence‑based care to patients with multiple chronic conditions across Arizona. You will join a multidisciplinary team, build long‑term relationships, and operate within a value‑based care model to improve outcomes.
The role emphasizes comprehensive wellness visits, remote telehealth support, and coordinated care with PCPs and specialists.
Qualifications
- Active and unrestricted NP license or PA license.
- Board certification (ANCC/AANP for NP; NCCPA for PA).
- Current DEA certificate or eligibility to obtain.
- Minimum 2 years of direct patient care experience.
- Field‑based role requiring travel to patient homes with reliable transportation.
- BLS certification supported during onboarding.
- Knowledge of HIPAA and OSHA regulations.
- Experience with EHR systems, preferably Athena or Salesforce.
Responsibilities
- Provide evidence‑based, patient‑focused care in patients' homes.
- Perform comprehensive assessments and follow‑up visits.
- Educate patients and caregivers on disease management and testing.
- Participate in virtual integrated care team meetings.
- Coordinate care with PCPs, specialists, and ancillary services.
- Complete documentation accurately and promptly.
- Occasional travel to patient homes may be required.
