Job summary

Adobe Population Health seeks a Nurse Practitioner to deliver hybrid care through telehealth and in-home visits across Phoenix and Maricopa County. Qualifications

• Master’s degree in nursing (MSN) required.
• FNP specialization required.
• Current and unencumbered NP license in AZ required.
• CPR and BLS certification required.

Responsibilities

• Conduct in-home assessments including physical, psychosocial, environmental, and safety evaluations.
• Perform telehealth visits to manage acute and chronic conditions with focus on respiratory health and population health needs.
• Develop patient-centered care plans and educate patients and caregivers on disease management and preventive strategies.
• Ensure HIPAA compliance and maintain accurate documentation in EHR systems.
• Travel up to 75–90% across designated regions for in-home visits.
