Qualifications

• Master’s degree in Nurse Practitioner or accredited PA program required.
• Current Arizona Nurse Practitioner license or PA-C in good standing.
• Comfort with initiating goals of care and end-of-life discussions.

Responsibilities

• Provide and coordinate primary care to members in their residence.
• Conduct physical/wellness exams and assessments for timely care.
• Collaborate with case management team to ensure necessary services.

Education
