CVS Health, in collaboration with Signify Health, is seeking a Full Time In-Home Physician to perform comprehensive health evaluations in patients' homes across Canton and surrounding areas.

The role involves 40 hours per week, flexible travel within licensed states, and virtual visits as needed, using an iPad-based workflow. Providers require active MD/DO licenses and multi-state eligibility; in-home care aims to bridge gaps in care and improve outcomes.

Formación

• Active, unrestricted license in the coverage area (OH and other states).
• Multi-state licenses are preferred to support travel across states.
• Experience with in-home health evaluations and care coordination is desirable.

Responsabilidades

• Provide in-home health evaluations, including medical history review and physical exam.
• Visit patients at home, typically ~45 minutes, with optional diagnostic tests as ordered.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Use iPad-based clinical workflow; collaborate with care teams to connect patients to appropriate care.
