Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations in patients' homes. You’ll perform medication and medical history reviews, physicals, and optional diagnostic tests as ordered by the health plan.

You’ll join a national network of clinicians and may travel locally within licensed states, with virtual visits as needed and no requirement to prescribe or alter treatments.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Bring your heart into every visit, joining a national network of purpose-driven clinicians.
• Visit millions of people nationwide to connect them with the right care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, DEE, bone density screening).
