Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide in-home health evaluations, engaging with members in their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional tests ordered by the health plan. You’ll use an iPad and travel within licensed states as needed.

Formación

• Active, unrestricted MD/DO license(s) in coverage area(s).
• Able to cover Erie - Region; travel locally and within licensed states.

Responsabilidades

• Provide in-home health evaluations for plan members (~45 minutes per visit).
• Perform medication and medical history review, physical evaluation, and ordered diagnostic tests as applicable.
• Document findings via an intuitive clinical workflow; coordinate with care teams and plan providers.
