Job summary

Signify Health, part of CVS Health, seeks a Per Diem Physician to perform In-Home Health Evaluations. The role involves engaging with patients in their homes, reviewing medications and medical histories, performing a physical evaluation, and coordinating with health plans as needed.

Visits last about 45 minutes, include optional tests, use an iPad with a clinical workflow, and do not require prescribing or altering treatment.

• Multi-state licenses (preferred).
