Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Physician to perform in-home health evaluations for members in their own homes. You will conduct a 45-minute visits, review medications and medical history, perform a physical evaluation, and follow a clinical workflow using an iPad.

No prescribing or test ordering is required. You must hold an active medical license(s) and be able to travel locally and within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home Health Evaluations.
• Engage with patients in their homes to bridge gaps in care.
• Contribute to a patient-care plan in collaboration with health plans.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
