Job summary

Signify Health, part of CVS Health, seeks a Part-Time Physician to perform in-home health evaluations at patients' homes. You will connect patients with the right care, review medications and history, and provide preventive services via an intuitive workflow.

The role supports local travel, virtual visits, and coordination with health plans, with a 29.5-hour work week and a base pay of $105/hour.

Qualifications

• MDs and DOs with active, unrestricted licenses in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations to patients in their homes.
• Bridge gaps in care by coordinating with health plan and care teams.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
