Job summary

Signify Health, part of CVS Health®, seeks a Part Time Physician to provide in-home health evaluations. You will meet patients in their homes, review medical history and medications, and perform physical assessments with a focus on bridging gaps in care.

This role requires active medical licensure and potential multi-state eligibility. Travel locally and within licensed states is expected; virtual visits are also part of duties.

Qualifications

• Active, unrestricted medical license in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for members.
• Discuss health history and medications with patients.
• Perform physical evaluations and document findings.
• Coordinate with health plans to suggest next steps and referrals.
• Conduct virtual visits as needed.
