Job summary

Signify Health, part of CVS Health, is seeking a Full-Time Physician to perform in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. You will conduct visits lasting about 45 minutes, including a medication and medical history review, a physical evaluation, and any additional tests ordered by the health plan.

You will use an iPad with a streamlined clinical workflow and are not required to prescribe medicines or modify treatment.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.
• Able to cover the following primary locations: Augusta, ME.

Responsibilities

• Conduct in-home health evaluations lasting about 45 minutes with history review and physical exam.
• Provide diagnostic & preventive services as indicated by the health plan.
• Travel locally across licensed states and conduct virtual visits as needed.
