Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of the Physical Therapist and Clinical Manager. The role focuses on delivering a patient-centered plan to improve strength, mobility and independence for geriatric and other patients in a home setting.

Responsibilities include implementing therapy plans, documenting progress, participating in staff meetings, and ensuring safety with infection control and equipment

Qualifications

• Licensed Physical Therapist Assistant credential required.
• CPR certification is required.
• Experience collaborating with a healthcare team.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapies and other modalities as prescribed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Report equipment issues, supply shortages, or changes in staff or client status.
• Apply infection control and universal precautions during patient care.
