Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to join our caring team. You will administer the physical therapy treatment plan in patients' homes, under the supervision of a Physical Therapist, helping patients regain strength and independence.

You’ll document care, participate in team education, and ensure safety and infection control. The role requires an associate degree in physical therapy, valid state license, CPR, and reliable transportation.

Qualifications

• Associate degree in Physical Therapy from an American Physical Therapy Association.
• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes.
• Administer non-complex active and passive manual therapies and modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.

Skills

Current state license to practice PT

Valid driver's license & reliable auto

Current CPR certification

Experience collaborating with a care团队
