CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home therapy under the guidance of the Physical Therapist. You will work onsite in patients' homes, using evidence-based methods to improve function and independence within a compassionate, patient-centered environment.

The role emphasizes collaboration with a multidisciplinary team, timely documentation, and adherence to infection control practices. A valid state PTA license and driving needs are required.

Qualifications

• Must hold current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical, emotional and social changes.
• Read, write and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Perform non-complex manual therapies and modalities as prescribed.
• Document client records and submit on time each week.
• Attend educational meetings and coordinate department in-services.
• Report equipment failures or supply issues to PT or Branch Director.
• Apply infection control and universal precautions in patient care.
