Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes. You will implement the plan developed by the Physical Therapist and collaborate with a healthcare team.

Requirements include a PT license, valid driver’s license with insurance, CPR, and English fluency. Preference for 1 year PTA experience. Pay per visit ranges $41.00–$57.00; emphasis on safety and patient-centered care.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the PT treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in patient care.
