CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes. You will implement the PT-created plan, focusing on functional improvement and compassionate, patient-centered care.

Required: PTA license in state, valid driver's license, CPR, and teamwork with healthcare professionals. 40-hour work week, compensation $64,000–$87,500/year, plus comprehensive benefits.

Formación

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.

Responsabilidades

• Administer the PT treatment plan to patients in their homes as developed by the Physical Therapist.
• Perform non-complex manual exercises, therapeutic massage, heat/cold, and electrical modalities as prescribed.
• Document client records and submit on time weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in care.
