Job summary

CenterWell Home Health seeks a licensed Physical Therapy Assistant to deliver in-home therapy under the supervision of a Physical Therapist. You will implement the treatment plan, document progress, and coordinate with the care team to support functional independence.

32 hours weekly; competitive pay and benefits in a patient-centered setting. Valid state license, CPR, driver's license, and English fluency required; prior geriatric care experience preferred.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex manual therapy exercises and modalities.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in care activities.
