Job summary

Nurse Next Door Coeur d'Alene is seeking a Physical Therapy Assistant to provide in-home PT services across North Idaho. You will implement treatment plans, monitor patient progress, and collaborate with supervising PT to ensure high-quality care in patients' homes.

The role requires PTA licensure, accredited program, and passing national exam; a valid driver's license and CPR are required. Home health experience is preferred, and you will document visits using the Axxess EHR system.

Qualifications

Licensed PTA in the practicing state or eligible.
Graduate of an accredited PTA program.
Passed the national PTA exam.
Experience in home health preferred.
Valid driver's license and transportation.

Responsibilities

Implement treatment plans ordered by the physician for all patients on caseload.
Review caseload with supervising PT regularly.
Evaluate assessment findings and communicate patient status.
Inform team and physicians about changes in patient status.
Educate patients and families on physical therapy modalities.
Deliver PT modalities appropriate to patient status.
Ensure patient safety and reduce errors.
Participate in case conferences about care plans.
Identify environmental risk factors and intervene.
Maintain knowledge of community resources for transfers and discharge.
Complete visit notes in the EHR promptly.
