Job summary

The Lakes Home Care, Inc. is seeking a dedicated Physical Therapist Assistant to join our home care team. You will assist Physical Therapists in developing and implementing treatment plans, help patients regain movement, and monitor progress during sessions.

You will educate patients and families on at-home exercises, document outcomes, and collaborate with other healthcare professionals to ensure comprehensive, patient-centered care in a home setting.

Qualifications

• Previous experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
