Job summary

The Lakes Home Care, Inc. is seeking a dedicated Physical Therapist Assistant to join our team. The ideal candidate will support physical therapists in developing and implementing treatment plans for patients across home care settings.

You will help patients regain movement, educate families on home exercises, document progress, and collaborate with healthcare professionals to ensure comprehensive, compassionate care in patients' homes.

Qualifications

• Previous experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
• Educate patients and their families on at-home exercises and care routines
• Collaborate with healthcare professionals to ensure comprehensive patient care
