Job summary

Qualifications

• Must have a valid license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

• Attends and contributes to patient care and required meetings as a representative of physical therapy.
• Adheres to department and patient treatment schedule and documentation requirements.
• Works with rehab team to implement programs for each patient.
• Assists the Physical Therapist during tests, evaluations, and complex procedures.
• Instructs patients and caregivers in therapy techniques under supervision.
• Maintains timely documentation for all patients treated.
• Cleans, maintains, and reports equipment needs.
