Job summary

Aveanna Healthcare is seeking a Physical Therapist in Minnesota for salaried, home health visits across Wabasha, Winona, Buffalo and surrounding communities.

Monday–Friday schedule with interdisciplinary collaboration and ongoing assessments to drive patient mobility and outcomes, including plan of care and discharge planning. Licensure in MN/WI required, CPR and auto insurance proof provided.

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in MN/WI.
• Valid driver’s license, CPR and proof of automobile insurance.

Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
• Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and outcomes.
• Delivery of physical therapy services to comply with prescribed physician orders.
• Maintains plan of care by documenting client performance, modifying goals and treatment as needed, and determining discharge plans.
• Discharge planning and education of patient and caregivers on home exercise program.
• Participates in coordination of services with health care team, physicians, parents/guardians, nurses, etc.
• Supervises PTA/HHA as applicable and completes OASIS assessments where state practice allows.
• Maintains effective communication between staff and family; serves as a resource consultant for clinical staff.
