Job summary

Aveanna Healthcare is seeking a Physical Therapist to join our home health team serving Ely, Minnesota and surrounding areas. You will assess, teach, and deliver therapy interventions directed by the Physician and within the home health plan of care, collaborating with the interdisciplinary team to optimize patient well-being.

In this full-time position, you will perform initial and ongoing mobility and functional assessments, develop treatment plans, document outcomes, and coordinate discharge

Qualifications

• Degree in Physical Therapy from an accredited university/program.
• Current, unrestricted state license as a Physical Therapist in the state of practice
• Valid CPR

Responsibilities

• Perform initial and ongoing mobility, coordination, strength and functional assessments.
• Provide written evaluation and plan of care defining PT needs, goals and progress.
• Deliver physical therapy services according to physician orders and home health plan of care.
• Maintain plan of care with timely documentation and discharge planning as appropriate.
• Communicate effectively with the health care team, patients and families.
