Job summary

Aveanna Healthcare is seeking a licensed Physical Therapy Assistant to provide in-home skilled therapy in the Rochester, MN area. This full-time role operates Monday through Friday, collaborating with an interdisciplinary team to implement physician plans of care and optimize patient outcomes.

You will educate patients and caregivers, document progress, and ensure safe, effective treatment while maintaining confidentiality and professional standards.

Qualifications

• Must graduate from an accredited PTA program and hold a current license.
• Valid CPR certification and responsibility to maintain licensure.
• Ability to provide skilled therapy in home health settings.
• Strong communication with patients and caregivers.

Responsibilities

• Provides direct physical therapy under supervision and in accordance with the plan of care.
• Communicates with and updates the patient and caregiver regarding the plan of care.
• Documents assessments and progress notes to ensure continuity of care.
• Reports observations to designated supervisor and physician as appropriate.
• Plans discharge instructions and coordinates with family and caregivers.
