Job summary

Assured Home Health in Oregon seeks a full-time Physical Therapist to assess patients, develop and implement therapy plans, and coordinate care with the health team. This role emphasizes mobility, pain management, and improving functional status in home-based settings.

Eligible candidates will have an Oregon PT license, a valid driver's license with reliable transportation, and CPR certification (or able to obtain within 90 days).

Qualifications

• Current Physical Therapy licensure in state of Oregon.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in development of the plan of care.
• Provides services within the scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family on plan of care, limitations, and progress.
• Completes evaluations, progress notes, and reports outcomes as per policy.
• Reports outcomes of evaluation, goals, and projected frequency of care.
