Optum is hiring a Physical Therapist for home health in Oregon. The role involves assessing patients, developing and implementing treatment plans, and coordinating with the health care team to improve mobility and pain management.

The candidate must hold an Oregon PT license, have access to reliable transportation, and ideally CPR certification. A strong emphasis is placed on documenting progress and adhering to state guidelines.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Current Physical Therapy licensure in state of Oregon.
• Valid driver's license and vehicle insurance or reliable transportation

Responsabilités

• Evaluates patient functional status and needs and assists in therapy plan of care development.
• Provides PT services within scope of practice and plan of care in coordination with health care team.
• Observes and reports patient responses to treatment and progress to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Completes progress notes after each visit and documents progress per policy.
• Develops PT plan of care within state guidelines and reports outcomes.
• Ensures care regardless of patient demographics.
