Job summary

Assured Home Health, part of LHC Group within the UnitedHealth family, is hiring a Physical Therapist for home health in Oregon. You will assess patient needs, determine treatment plans, and implement interventions to improve mobility and function, while coordinating with the health care team.

Requirements include current Oregon PT licensure, a valid driver's license and reliable transportation; CPR certification or ability to obtain within 90 days.

Qualifications

• Current Physical Therapy licensure in Oregon.
• Current driver’s license and access to a dependable vehicle.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient’s functional status and physical therapy needs.
• Assists physician in development of the therapy plan of care.
• Provides services within state scope of practice and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
