Job summary

UnitedHealth Group's Assured Home Health seeks a Physical Therapist to assess patients in Grants Pass, OR and develop treatment plans to improve mobility and function in home health care.

Responsibilities include evaluating patients, planning care, delivering interventions, and coordinating with the health care team. A current Oregon PT license and a valid driver's license are required; a 10,000 sign-on bonus is available for eligible full-time candidates.

Qualifications

• Current Physical Therapy licensure in the state of Oregon.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates patient's functional status and PT needs; assists physician in plan of care.
• Provides PT services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and care team about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
